IMM5483: Document Checklist
http://www.cic.gc.ca/english/pdf/kits/forms/imm5483e.pdf
IMM5483: Document Checklist
http://www.cic.gc.ca/english/pdf/kits/forms/imm5483e.pdf
IMM1294: Application for Study Permit
http://www.cic.gc.ca/english/pdf/kits/forms/IMM1294B.PDF
IMM5645: Family Information
http://www.cic.gc.ca/english/pdf/kits/forms/IMM5645E.PDF
http://www.canadainternational.gc.ca/hong_kong/assets/pdfs/st-additionalforms.pdf
網頁上有說 (only for persons also requiring Temporary Resident Visas)
Application of Temporary Resident Visa
Statutory Declaration of Common-law Union
IMM5646: Custodianship Declaration
IMM5476: Use of Representative
Letter of Acceptance from the Educational Institution
Proof of Financial Support
CAQ
Fee Payment
Valid Passport
根據 http://www.canadainternational.gc.ca/hong_kong/visas/study_kit-trousse_etudiant.aspx?lang=eng&view=d
Persons who do not require Temporary Resident Visas for Canada can submit photocopies of the biographical pages of their passport rather than the actual passport.
Two Photos
Additional Documents
Study Plan
根據 http://www.canadainternational.gc.ca/hong_kong/assets/pdfs/study-app.pdf
You must attach a study plan (1-2 pages in length) to the application. It should address the following issues:
a. Why you wish to come to Canada to study at the school and in the program for which you have been
accepted;
b. What is your overall educational goal;
c. Why you are not pursuing less expensive alternatives in your home country (What are those
alternatives? Have you investigated them?);
d. What ties do you have to your home country that will lead you to return home following your course
of studies?
e. How will this Canadian diploma/degree increase your chances in the labour market in your home
country?
